Service Description

If you are diagnosed with diabetes, you may have coverage for therapeutic footwear at little or no out-of-pocket or up-front costs. You will need the following: 1. Your Prescription for footwear. Bring or fax to 270-747-8779 2. Bring Your Insurance Cards 3. Insurance accepted: - Medicare - Mass Health - Blue Cross Blue Shield - Harvard Pilgrim - Tufts Healthcare - Cambridge Pace - Pace Harbor Health - Commonwealth Care Alliance - Senior Whole Health - Tricare - Unicare - United Healthcare (with prior approval) 4. Criteria - History of partial or complete amputation of the foot - History of previous foot ulceration - Peripheral neuropathy with evidence of callus formation - Foot deformity - Poor circulation 5. Documentation Identify the following: 1. Practitioner who manages your diabetes 2. Practitioner who examines your feet (if different). For fastest service, call their offices and ask that recent progress notes be faxed to 270-747-8779. Or, after your first visit, with your permission, we will request required documentation. 6. Wear Socks 7. Bring Braces that you plan to wear with the shoes.
